NCR details the defect or non-conformity, includes a root cause analysis (if it is major NCR), outlines corrective actions, and documents the eventual closure of the issue. This report serves as crucial audit evidence, highlighting the importance of maintaining quality standards within an organization.

A Non-Conformity Report (NCR) is essential for documenting deviations from quality standards. These deviations are categorized into two types:

Understanding the distinction between Major and Minor NCRs is crucial for maintaining quality and safety standards in any organization.

– Major NCRs: These involve critical safety issues, significant system failures, or impacts on customers. They necessitate immediate corrective action and often require halting processes to address the concerns. Major NCRs do require a preventive action based on root cause analysis in addition to corrective active to remedy the situation.

– Minor NCRs: These are isolated lapses that have low impact. While they require correction, they do not immediately compromise the integrity of the product. Minor NCRs are not normally not required a preventive action. However, if several minor NCRs of the same nature reoccurs repeatedly. It should be treated as a major NCR with full preventive action formulated to prevent recurrence based on root cause analysis.

Root Cause Analysis (RCA) is a structured and systematic approach aimed at identifying the underlying causes of problems or incidents to prevent their recurrence.

Root Cause Analysis goes beyond addressing superficial symptoms and focuses on understanding why an event occurred rather than merely detailing what happened.

By analysing systems, human error, and processes, RCA facilitates the implementation of lasting solutions.

One of the most popular method used for RCA is 5 Whys method.

The 5 Whys is a straightforward and iterative troubleshooting technique that helps identify the root cause of a problem by repeatedly asking “Why?”

Typically, this process involves asking the question around five times, allowing individuals to drill down from a symptom to its underlying cause. This method can be an effective tool for problem-solving in various contexts.
